Requirements
- Family must meet current year income guidelines and show proof of income
- Child must meet age requirements and be a Dallas county resident
Documents
- Proof of income (Income Tax, SSI, TANF, Child Support or other requested by Family Advocate)
- Current physical, dental exam and shot record prior to enrollment
- Completed application form (located at the center)
- Child’s birth certificate, social security card, health insurance
- If child has been diagnosed as having a disability, submit a copy of professional evaluation or medical record identifying the disability
